Concierge Home Health Licensed Vocational Nurse (LVN)
Location: Los Angeles, CA
Position Type: Per Diem / On-Call / Part Time
Compensation: Starting at $40/hour (varies by tier)
Position Overview: The Home Health Nurse is responsible for ensuring that TheKey Concierge Nursing Services delivers a quality care experience to our clients and ensures the client care plans for each client are continuously kept up-to-date by in-person or virtual reassessment and reports. This may include physical, emotional and social well-being for our clients, often creating life enriching opportunities and allowing as much independence as possible to be retained. The Home Health nurse is responsible to ensure we are the provider of choice. The Home Health nurse is also integral to ensuring smooth operations and client satisfaction, supporting long-term retention of cases, and working in partnership with colleagues in the immediate team environment and in the larger community.
Responsibilities
Essential Duties and Responsibilities
Client Onboarding & Assessment
Initiate Prompt Client Outreach: Proactively connect with new clients immediately upon assignment to schedule initial meetings and establish strong collaborative relationships with clients and their families.
Conduct Nursing Assessments: Perform comprehensive, in-person initial nursing assessments, reassessments, and quality assurance visits—including home safety evaluations, daily routines, preferences, and formal/informal support systems.
Develop Tailored Care Plans: Translate assessment findings into individualized Care Plans that incorporate preventative and rehabilitative nursing procedures tailored to the client's unique needs.
Care Coordination & Clinical Management
Provide Direct Nursing Care: Deliver skilled nursing services in full accordance with the established plan of treatment.
Coordinate Services: Oversee and coordinate all care and services provided to ensure a seamless client experience.
Monitor Changes in Condition: Perform regular home visits and timely reassessments during condition changes, updating care plans and communicating promptly with physicians, practitioners, interdisciplinary team members, and The Key Concierge Nursing Services team.
Educate Clients & Families: Provide ongoing instruction and health education to clients, family members, and care staff.
Client Retention & Experience
Build Rapport & Engagement: Serve as a highly visible, trusted point of contact to maintain a exceptional experience for clients and their support networks.
Support Client Retention: Drive long-term retention through dedicated hands-on attention, client appreciation initiatives, and proactive communication.
Onboarding Support: Provide dedicated assistance and close oversight during the launch of new client cases.
Compliance, Documentation & Operations
Ensure Regulatory Compliance: Maintain full compliance with state home care regulations, agency standards, and visit documentation requirements.
Timely Documentation: Prepare accurate clinical notes and submit documentation prompt according to agency policy.
Responsive Communication: Respond to inquiries, calls, and messages within agency-established timeframes.
Rate Reviews: Monitor and review client service rates as care needs evolve.
General & After-Hours Support
On-Call Availability: Provide on-call and after-hours support as required.
Additional Duties: Perform other duties as assigned to support clinical and business goals.
Qualifications
Required Qualifications, Education & Certifications
Licensure: Current, unencumbered Licensed Vocational Nurse (LVN) license in the state of employment (active status must be maintained as a condition of employment).
Experience: Minimum 2 years of experience in healthcare, eldercare, home care, or a related clinical field preferred.
Technical Skills: Computer proficiency with the ability to complete and submit accurate, timely clinical documentation in the electronic health record system.
Interpersonal Skills: Strong customer service, communication, and conflict-resolution abilities.
Driver Requirements: Valid driver’s license and proof of current auto insurance.
