Responsibilities
If you're a licensed clinician who believes meaningful change happens beyond the walls of an office, this role is designed for you. As a Family Centered Treatment (FCT) Therapist at Health Connect America, you'll provide high-quality, evidence-based clinical services to children, adolescents, and families—delivered where it has the greatest impact: in their homes and communities.
What You'll Do
As an FCT Therapist, you'll lead clinical care for a caseload of families, using a structured, research-supported model to drive lasting outcomes. Your work will be hands-on, collaborative, and rooted in clinical excellence. Day to day, you can expect to:
Deliver Family Centered Treatment services in home and community-based settings, ensuring care is accessible and relevant to each family’s environment
Guide families through the four phases of the FCT model, facilitating progress through structured interventions and fidelity-driven practices
Develop, implement, and maintain individualized Service Plans in collaboration with the Treatment Team, ensuring alignment with authorization and clinical requirements
Coordinate and lead Treatment Team meetings, partnering with medical, behavioral health, school, and community providers to ensure integrated, whole-person care
Provide psychoeducation and therapeutic interventions aligned with each family’s goals and clinical needs
Support families in setting and achieving weekly goals, reinforcing skill-building and sustainable behavior change
Maintain timely, accurate clinical documentation and FCT fidelity records within the Electronic Health Record
Oversee assessments, discharge planning, and aftercare coordination, connecting families to community resources that support long-term success
What to Expect
This is a community-based clinical role requiring a flexible schedule to meet families when they are most available. The position includes 24/7 on-call responsibility to support crisis needs across your caseload. While urgent situations are not the norm, your availability provides critical stability and continuity of care for the families you serve.
As a licensed clinician, you may also provide supervision and will be responsible for maintaining active credentials, meeting ongoing training requirements, and ensuring compliance with all regulatory and organizational standards. Your NPI will be utilized for billing services delivered under your supervision.
Your Path to FCT Certification
FCT Certification is required for this role and must be completed within one year of hire. Health Connect America provides structured support through required checkoffs, e-learning modules, and ongoing clinical guidance. This certification strengthens your clinical skillset and positions you as a specialist in delivering high-impact, family-centered care.
