We are hiring for Full-Time, Part-Time, and PRN opportunities, with different territory options depending on the schedule you're looking for.

What You'll Do
Working under the direction of the RN and physician-established plan of care, you will:

Provide skilled nursing visits in patients' homes

Perform focused nursing assessments
-
Monitor vital signs and changes in patient condition
-
Provide wound care and treatments as ordered
-
Assist with medication management and medication education
-
Reinforce disease-management education
-
Monitor treatment effectiveness and patient response
-
Educate patients, families, and caregivers
-
Communicate changes in condition promptly to the RN and appropriate members of the care team
-
Follow physician orders and established plans of care
-
Collaborate with the interdisciplinary home health team
-
Complete accurate and timely clinical documentation using WellSky
What We're Looking For
-
Active Maryland LPN License Required
-
Graduate of an approved Practical Nursing program
-
Reliable transportation
-
Strong clinical assessment and communication skills
-
Excellent time-management and organizational abilities
-
Ability to work independently in patients' homes

Full-Time candidates must be able to cover all of Montgomery County
-
Home Health Experience Preferred, but Not Required
-
WellSky Experience is a Plus
