Reliance Hospice and Palliative Care is actively seeking an experienced Community Health Worker to support patients and families throughout the Los Angeles area.
This is a field-based position for a highly experienced, professional, and compassionate individual who is comfortable spending most of the day traveling between patient homes, healthcare facilities, and referral sources and meeting directly with patients and families.
Hourly compensation starts at $25/hour and up, depending on experience.
The ideal candidate will have a strong healthcare background, excellent customer service and interpersonal skills, and experience communicating with patients, families, healthcare professionals, and referral partners. Hospice or palliative care experience, as well as healthcare outreach or sales experience, is required.
JOB DESCRIPTION SUMMARY
The Community Health Worker is responsible for providing patients and families with clear explanations and education regarding hospice and palliative care benefits, helping ensure timely access to services, and increasing community awareness of Reliance Hospice & Palliative Care.
The CHW will also assist with communication between the agency and referral sources and develop relationships within the healthcare and local community.
Responsibilities include:
- Meet with patients and families in homes, healthcare facilities, and other community settings.
- Explain hospice and palliative care services and benefits in a clear, compassionate, and professional manner.
- Provide excellent customer service and serve as a positive representative of Reliance Hospice & Palliative Care.
- Build and maintain relationships with referral sources, healthcare professionals, community organizations, and families.
- Conduct community outreach and education for healthcare professionals, civic leaders, and members of the public.
- Assist with communication between patients, families, referral sources, and the agency.
- Travel throughout the assigned Los Angeles service area as needed.
- Demonstrate compassion and professionalism when working with patients and families facing life-limiting illness, death, or bereavement.
- Independently coordinate activities while maintaining strong organization and follow-through.
POSITION QUALIFICATIONS
- High school diploma or GED required; Associate's or Bachelor's degree preferred, or equivalent work experience.
- At least 2 years of hospice and/or palliative care experience required.
- Previous healthcare sales, business development, community outreach, or referral development experience required.
- Strong customer service and interpersonal skills.
- Excellent communication, negotiation, public relations, and problem-solving skills.
- Ability to communicate tactfully and compassionately with patients, families, referral sources, and community partners.
- Professional, confident, and comfortable working in the field and meeting with patients and families throughout the day.
- Ability to work independently, organize activities, and manage multiple priorities.
- Bilingual in Spanish is preferred.
- Reliable transportation and willingness to drive to patient homes, facilities, and referral locations.
- Ability to meet the physical requirements of the position.
