Established as a community non-profit in 1972, The Center provides targeted programs and services to the full diversity of the San Diego LGBTQ community. The mission of The Center is to enhance and sustain the health and well -being of the lesbian, gay, bisexual, queer, transgender, nonbinary, immigrant, and HIV communities for the betterment of our entire San Diego region. Our vision is to create a Center and Center culture where every person feels and knows that they are welcomed, valued, and supported. The Center is sex-positive and provides advocacy and critical health and social support services to support our community members through all stages of life, focusing on our deeply held values of equity, social justice, and client-centered, trauma-informed care. The Center currently has over 100 staff working at 7 facilities across San Diego County.

POSITION SUMMARY

Under the supervision and support of the Transgender and Nonbinary Services Manager, the Trans(C)ending Case Manager provides comprehensive, trauma-informed, gender-affirming, and culturally responsive case management services to Black and Latina transgender women, prioritizing participants enrolled in the Trans(C)ending initiative research project with UCSD. This position supports Black and Latina transgender women experiencing barriers to health care, HIV prevention and treatment services, housing stability, economic security, behavioral health services, and other social determinants of health. This is a temporary position scheduled to end on June 30, 2028.

Using a participant-centered and strengths-based approach, the Case Manager assists participants in identifying goals, navigating complex systems of care, accessing gender-affirming services, and addressing barriers to overall health and well-being. The position works collaboratively with community partners and research collaborators to support participant engagement, retention, and linkage to services while ensuring confidentiality and compliance with organizational, funder, and research requirements. The ideal candidate possesses experience working with transgender and BIPOC communities, demonstrates cultural humility and responsiveness, and has knowledge of the unique challenges impacting Black and Latina transgender women. Spanish-language proficiency is strongly preferred.

PRIMARY ROLES & RESPONSIBILITIES:

Client Support:

  • Provide individualized case management services, including assessment, service planning, goal setting, resource navigation, advocacy, and ongoing follow-up for program participants.
  • Develop and maintain participant-centered care plans that address barriers related to healthcare access, HIV prevention and treatment, housing stability, employment, income support, behavioral health, legal services, transportation, and other identified needs.
  • Conduct engagement and retention activities to support participation in the Trans(C)ending initiative and promote continuity of care.
  • Coordinate referrals and linkages to culturally responsive, gender-affirming, and trauma-informed services, both within The Center and through external community providers.
  • Assist participants in navigating public benefits systems, health insurance enrollment and utilization, medical referrals, prior authorizations, appeals processes, and other healthcare-related systems.
  • Advocate on behalf of participants to reduce barriers to care and improve access to services that support health, safety, stability, and self-determination.
  • Maintain participant confidentiality and comply with all applicable privacy, HIPAA, and organizational requirements.
  • Provide crisis intervention, safety planning, and stabilization support as appropriate, including coordination with internal and external service providers.

Outreach and Event Planning:

  • Attend Trans Health Project Drop-In space, when appropriate, to meet with program participants, schedule appointments, and connect participants with resources.
  • Develop and maintain positive working relationships with community partners, healthcare providers, social service agencies, and other stakeholders serving transgender communities.
  • Collaborate with Center staff, research partners, and community organizations to support implementation of Trans(C)ending program activities and participant support services.
  • Participate in program meetings, case conferences, and collaborative planning activities as required.

Administration, Data Entry, and Reporting:

  • Maintain timely, accurate, and complete case notes, service records, participant files, and referral documentation in designated data management systems.
  • Collect and document participant information, service utilization, outcomes, and other required program data in accordance with organizational, funder, and research protocols.
  • Support data quality efforts by ensuring participant records are accurate, complete, and submitted within required timeframes.
  • Prepare documentation and reports necessary for program monitoring, quality assurance, grant compliance, and evaluation activities.
  • Participate in audits, monitoring activities, and program evaluation efforts as requested.
  • Support implementation of program activities, workshops, support groups, and community engagement opportunities

Required Qualifications:

  • Education and/or Experience: 1 year of experience providing case management services to transgender, nonbinary, and gender expansive communities.
  • Language Skills: Fluency in English required.
  • Computer/Office Equipment Skills: Must be comfortable working in PC/Windows-based environments. Must have strong proficiency with Microsoft Office.
  • Other Qualifications:
    • Demonstrated ability to manage multiple tasks with shifting priorities.
    • Must demonstrate competence in working with diverse populations.
    • Must demonstrate competence in using strength-based, trauma-informed, and motivational interviewing techniques.
    • Alignment with the organization's mission and values, including fostering a welcoming and inclusive environment

preferred QUALIFICATIONS:

  • Language: Fluency in Spanish.
  • Experience: 2 or more years of experience providing case management services to transgender, nonbinary, and gender expansive communities. 1 year or more of experience providing case management services to people living with mental illness and/or substance use disorders.
  • Other Qualifications: Knowledgeable about current issues related to the LGBTQIA+ community.

COMPENSATION & BENEFITS:

  • Compensation: The pay range for this position is $23.50 - $25.00 an hour and is commensurate with experience. This is a Full-Time, Temporary Non-Exempt position. This position scheduled to end on June 30, 2028.
