Lead. Coordinate. Optimize.

 

As a Centralized Intake Lead, you will ensure timely, accurate, and efficient handling of referrals from subscribing facilities while guiding and supporting a team of Centralized Intake Coordinators. You’ll oversee processes, reporting, scheduling, and quality monitoring to maximize efficiency and strengthen vendor relationships. Your leadership will help ensure patients are connected quickly and smoothly to the care they need.

 

As a Centralized Intake Lead, You Will:

  • Monitor referral vendors and oversee timely responses to all referrals/leads from subscribing facilities.

  • Supervise and support Centralized Intake Coordinators in managing acceptance, non-admission, and pending patient workflows.

  • Oversee process improvements, schedule management, reporting, and quality monitoring.

  • Provide training, user support, and troubleshooting for referral portal utilization.

  • Coordinate with branch, area sales, and operations leadership to ensure smooth implementation and vendor communications.

  • Maintain SharePoint and ensure timely follow-up communications.

  • Prepare reports to monitor portal activity and ensure data integrity.

  • Participate in evening/weekend on-call or overtime as required.

About You

Qualifications – What You’ll Bring:

 

  • High school diploma or equivalent 

  • 2–3 years’ experience in customer service, admissions, or related field.

  • Strong organizational skills with the ability to manage multiple priorities.

  • Excellent customer service skills with a collaborative and professional approach.

  • Ability to work in an interdisciplinary setting while maintaining organized business and clinical records.

  • Leadership mindset with experience supervising or training team members.

Preferred Qualifications (Not Required):

  • Bachelor’s degree in business, social work, psychology, or nursing
  • 1 year supervisory experience 
  • LPN/LVN certification 
  • Admissions or referral portal experience
