Clinical Resource Manager (RN)

As Clinical Resource Manager, you'll make a difference in residents' lives while guiding a dedicated clinical team.
Key Responsibilities:
Lead and coordinate resident assessments (MDS), ensuring compliance with federal and state regulations.
Champion quality care by developing individualized care plans and working closely with interdisciplinary teams.
Oversee Medicare and managed care referrals and ensure accurate documentation and care coordination.
Serve as a mentor, coach, and leader for clinical staff, promoting excellence in care delivery.
Act as Chairperson of the Interdisciplinary Care Plan Team and attend care conferences with residents and families.
Monitor clinical workflows, staffing levels, and resource utilization to meet resident needs efficiently and effectively.
What We’re Looking For:
Registered Nurse (RN) with a current, active license in this state.
AANAC certification (or willingness to obtain within 6 months).
Minimum 2 years of MDS experience preferred.
CPR certified and skilled in clinical documentation and assessment tools.
Strong leadership, communication, and collaboration skills.
Preferred Qualifications:
Bachelor of Science in Nursing (BSN).
Knowledge of Care Tracker, PPS Medicare criteria, and long-term care regulations.
