This leadership opportunity is ideal for an experienced rehabilitation professional who is passionate about improving the quality of life for older adults through exceptional therapy services, interdisciplinary collaboration, and program leadership.

The successful candidate will oversee rehabilitation operations while providing leadership to Physical Therapists, Occupational Therapists, and rehabilitation staff supporting medically complex geriatric patients. This role combines clinical leadership, operational oversight, team development, quality improvement, and patient-centered care within an innovative healthcare environment focused on helping seniors maintain independence and mobility.

Key Responsibilities

• Provide leadership and daily oversight for rehabilitation services supporting geriatric patient populations.
• Supervise, mentor, and develop Physical Therapists, Occupational Therapists, and rehabilitation support staff.
• Monitor clinical quality, patient outcomes, productivity, and departmental performance metrics.
• Collaborate with physicians, nursing, case management, and interdisciplinary care teams to develop individualized treatment plans.
• Ensure compliance with federal, state, and organizational rehabilitation standards and regulatory requirements.
• Assist with staffing, scheduling, performance evaluations, and professional development initiatives.
• Identify opportunities to improve rehabilitation workflows, operational efficiency, and patient satisfaction.
• Support program development designed to enhance mobility, independence, and functional outcomes.
• Maintain accurate clinical documentation and operational reporting.
• Foster a collaborative, patient-focused culture emphasizing safety, quality, and compassionate care.

Required Qualifications

• Current licensure as a Physical Therapist (PT) required.
• Previous leadership experience supervising rehabilitation or therapy teams.
• Experience managing rehabilitation services within a geriatric, post-acute, skilled nursing, outpatient rehabilitation, or similar healthcare setting.
• Strong understanding of rehabilitation operations, regulatory compliance, and quality improvement initiatives.
• Excellent leadership, coaching, and team development abilities.
• Strong communication and relationship-building skills with interdisciplinary healthcare teams.
• Demonstrated ability to manage multiple priorities while maintaining exceptional patient care standards.
• Must currently reside within commuting distance of Ontario, California, or be prepared to relocate independently before employment.
• Authorized to work in the United States.

Preferred Qualifications

• Leadership experience overseeing both Physical Therapy (PT) and Occupational Therapy (OT) programs.
• Experience caring for medically complex geriatric populations.
• Previous rehabilitation management experience within value-based or integrated healthcare environments.
• Experience leading quality improvement, operational performance, and clinical excellence initiatives.
• Strong understanding of Medicare, reimbursement, and rehabilitation documentation standards.
• Advanced leadership training, board certifications, or specialty rehabilitation credentials.
• Stable employment history demonstrating progressive clinical leadership and operational responsibility.

HireNow Package

• Compensation: $120,000–$160,000 annually
