Housing Case Manager

Description

Under the direction and guidance of the Chief Executive officer the housing Case Manager assists clients in moving from homelessness into permanent housing as well as providing homelessness prevention services to those at imminent level of homelessness. Housing Case Mangers assist clients in all aspects of obtaining and sustaining permanent housing. Through engagement with mainstream resources and services, the Case Manager guides clients through the implementation of the Individual/Household Service Plan with the goal of self-sufficiency and upward economic mobility.

The Housing Case Manager serves clients in both office and field settings to manage and monitor the housing stabilization/sustainability through direct client services, related documentation, as well as providing housing search and assisting clients in accessing other supportive services. Client integration into the community, and permanency in housing are paramount duties of the Housing Case Manager. The Housing Case Manager performs all administrative tasks involved in the review and maintenance of client files and related financial documentation of program participants. The Housing Case Manager routinely performs file reviews to ensure audit ready status.

Duties and Responsibilities:  

• Identify households at the greatest risk and determine what type of support they need to mitigate homelessness. 
• Establishing relationships with agencies and informing them of Mainstream services. 
• Recruit and maintain relationships with private landlords to participate in renting to clients  
• Assist clients to develop and monitor progress in meeting goals of Individual/Household Service Plan   
• Request payments request on behalf of clients such as, security deposits, utility deposits, and rent. 
• Provide advocacy and negotiation with landlords/rental managers to facilitate participation in housing programs  
• Collaborate with partners to implement the client’s full plan for obtaining and maintaining housing  
• Inspect all dwellings to ensure they meet Housing Quality Standards requirements. 

Requirements

• Certified Peer Specialist certification required 
• Bachelor of Social Work (BSW) degree preferred or a minimum bachelor’s degree in human service field and/or two years’ experience in the following:  
• Case management experience with adult clients 
• Experience/Understanding of issues affecting homeless individuals and families  
• Knowledge and practice using community resources and referrals  

Physical/Mental Requirements: 

• Prolonged periods of sitting at a desk and working on a computer 
• Must be able to lift up to 15 pounds at times.  

Additional Information: 

This position is an FLSA exempt position. The annual salary is $47,500.18 plus benefits. Security (HIPAA): Every employee is required to protect their computer log-on and password. You must secure your work area prior to leaving it unattended for an extended period of time. You are required to log-off of your computer and secure your work area prior to leaving at the end of the day. If you have a responsibility for closing or locking files, securing equipment, or locking the building and you do not comply with these requirements, you will be subject to reprimand and/or termination based upon the infraction. Environmental Working Conditions: This position includes both administrative and outreach responsibilities. Employees are frequently exposed to communicable diseases, toxic substances, medical cleaning and processing solutions which my included ionizing radiation plus other elements common in a clinical environment.
