Position Overview

The Intake & Enrollment Specialist serves as the first point of contact for patients seeking services at Fourth Street Clinic. This role plays a critical part in helping individuals access care by managing the intake and enrollment process and connecting patients to medical, dental, behavioral health, and case management services.

The Intake & Enrollment Specialist creates a welcoming and culturally responsive environment while ensuring patient information is collected, verified, and entered accurately into the clinic's electronic health record system.

Key Responsibilities

• Serve as the first point of contact for patients and visitors, providing a welcoming and respectful experience

• Conduct patient intake and enrollment processes for clinic services

• Collect and verify documentation related to patient eligibility and services

• Accurately enter patient information into the electronic health record (EHR) system

• Schedule appointments and coordinate referrals to internal programs and community partners

• Answer and manage calls through a multi-line phone system

• Maintain confidentiality and compliance with HIPAA and clinic policies

• Support a collaborative clinic environment focused on patient-centered care

Required Qualifications

• High school diploma or equivalent

• At least one year of customer service, administrative, or front desk experience

• Proficiency in Microsoft Office applications

• Experience using multi-line phone systems

• Strong organizational, communication, and customer service skills

Preferred Qualifications

• Bilingual in English and Spanish

• Previous experience working in a medical office or healthcare setting

• Experience using electronic medical records (EMR/EHR) systems

• Experience working with vulnerable or underserved populations

Compensation and Benefits

Starting pay begins at $18/hour and may vary based on experience.
