RN Health Specialist (Residential Services)

As an RN Health Specialist, you’ll use your clinical expertise to help people with intellectual and developmental disabilities live healthier, safer, more independent lives. This is a unique nursing role that combines clinical care, advocacy, care coordination, staff education, and leadership within a supportive, community-based environment.

You’ll build relationships with the people we support, their healthcare providers, residential teams, and families while helping ensure each person receives the right care at the right time.

What You’ll Do

• Champion the health and well-being of the people we support by monitoring health needs, identifying concerns, coordinating treatment, and advocating for quality outcomes.
• Coordinate medical care by communicating with healthcare providers, obtaining and maintaining medical orders, following up after appointments, and accompanying individuals to appointments as needed.
• Oversee medication management across assigned residential programs, including medication reviews, medication profiles, refills, storage, staff administration practices, and self-medication assessments.
• Develop individualized nursing and healthcare plans based on each person’s unique needs and changing health status.
• Provide clinical guidance and education to residential staff on health conditions, medication administration, medical procedures, and other health-related needs.
• Visit assigned residential programs regularly to assess health and medication systems, identify concerns, and provide ongoing clinical support.
• Partner with residential leadership and after-hours staff to ensure health concerns are communicated and addressed promptly.
• Ensure accurate documentation and regulatory compliance, including required health records, incident reporting, and OPWDD requirements.
• Provide clinical oversight to Medical Facilitators, as assigned, ensuring medical information, medication systems, appointments, documentation, and follow-up are managed appropriately.
• Participate in program development and organizational initiatives, including screening prospective individuals, meetings, committees, training, and other agency activities.

Requirements

What You’ll Bring

• Graduate of an accredited Registered Nursing program.
• Current, valid New York State RN license.
• Experience working with people with intellectual and/or developmental disabilities preferred.
• One year of supervisory experience preferred.
• Valid NYS Driver’s License with at least two years of driving experience.
• Reliable personal vehicle for business use.
• Acceptable motor vehicle record meeting agency insurance requirements.
