Palo Alto Medical Foundation is seeking a compassionate and skilled Physical Therapist to join our collaborative rehabilitation team.In this role, you will evaluate, develop, and implement individualized treatment plans that help patients restore mobility, reduce pain, build strength, and improve their overall quality of life following injury, illness, or medical conditions. You'll work closely with patients, families, and interdisciplinary care teams to deliver exceptional, patient-centered care while fostering trust through clinical expertise, empathy, and effective communication.
As a valued member of our rehabilitation team, you will have the opportunity to mentor and develop future clinicians by providing supervision to rehabilitation assistants and aides, as well as serving as a clinical instructor for students. Conduct comprehensive patient evaluations and develop personalized treatment plans.
Deliver evidence-based interventions designed to improve function, mobility, and independence.

Provide clinical guidance and supervision to assistants, aides, and students.

Job Description:

EDUCATION:
Graduate of an accredited physical therapy program
OR graduate of an accredited physical therapy program that can be demonstrated with a valid PT license
CERTIFICATION & LICENSURE:
PT-Physical Therapist
OR Will also consider approved license applicant or resident training program employee.
BLS-Basic Life Support Healthcare Provider
DEPARTMENT-SPECIFIC CERTIFICATION & LICENSURE:
Department: Home Health, DL-Valid Drivers License Class C
Department: Home Health, AUTO-Automobile Insurance
TYPICAL EXPERIENCE:
2 years of recent relevant experience
SKILLS AND KNOWLEDGE:
Critical thinking, complex problems solving, decisive judgement and ability to work independently.
Knowledge and application of professional practice and regulatory requirements.
Must be able to work in demanding work environment, organize multiple priorities completing work duties within expected timelines and requirements.
Computer and required technology proficiency/competencies.
Demonstrates effective and efficient professional communication (verbal & written) skills as well as interpersonal skills, conflict resolution, teamwork/collaboration, customer service and community relations abilities.
Able to retain and apply new knowledge & skills.
Keeps abreast of professional practice development and advancement.

Pay Range is $63.78 to $89.28 / hourThe compensation range may vary based on the geographic location where the position is filled.
