Overview
JOB PURPOSE: The Heart Transplant Coordinator is responsible for managing the care of Heart transplant recipients throughout the post-surgical phase. This role involves patient education, coordination of care with a multidisciplinary transplant team and ensuring compliance with regulatory and transplant program guidelines.
Responsibilities
Participates in the development, implementation, and maintenance of the left ventricular assist device (LVAD) program and the heart transplant program; to coordinate and manage the patient care of this high-risk patient population throughout the continuum.Qualifications
Education- Graduate from a nursing program Required
- 2 years of professional clinical experience Required
- Cardiovascular focus experience Preferred
- 1 year experience caring for LVAD patients and heart transplant patients Preferred
- RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
- ACLS Required
- BCLS - Basic Life Support Required
