Position Overview:
Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards.

Qualifications:
- Education: ADN/BSN from an accredited program.
- Licensure: Current and active New York State Registered Nurse (RN) license is required.
- Certification: Basic Life Support (BLS) certification is required;candidates must hold a current certification or be willing to obtain recertification prior to employment.
- Experience:
- Must have Pediatric experience.
- Minimum of 1.5 years of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity.
- Proven experience in hospital case management within an acute care environment is essential.
- Technical Skills: Proficiency in the EPIC Electronic Medical Records (EMR) system is preferred.
Key Responsibilities:
- Conduct utilization reviews for appropriate care level/coverage.
- Coordinate discharge planning and post-acute care transitions.
- Document care coordination in EPIC.
- Ensure compliance with payer guidelines and regulations.
- Collaborate with ED teams for rapid case management.
- Analyze resource utilization trends and recommend improvements.
