Human Touch Home Health is looking for a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities throughout Loudoun County, including Leesburg, Ashburn, Sterling, Purcellville, South Riding, Aldie, Middleburg, Hamilton, Lovettsville, and surrounding communities.

What You'll Do

• Complete speech-language pathology evaluations in patients' homes

• Develop individualized plans of care

• Treat speech, language, communication, cognitive, and swallowing disorders

• Provide dysphagia treatment and swallowing education as appropriate

• Help patients improve functional communication and cognitive skills

• Educate patients, families, and caregivers

• Monitor progress and adjust treatment plans as needed

• Coordinate care with nurses, physicians, therapists, and other team members

• Complete reassessments and discharge planning

• Maintain accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Speech-Language Pathologist License

• Graduate Degree in Speech-Language Pathology from an accredited program

• Current ASHA Certification (CCC-SLP) Preferred

• Valid Driver's License and reliable transportation

• Strong clinical and communication skills

• Ability to work independently in patients' homes

• Home Health Experience Preferred, but Not Required

• WellSky Experience is a Plus
