The Opportunity: Program Manager- Integrated Community Support Services Expansion
Job Type: Full-Time, Hybrid position, in-office and remote, which requires travel between different sites across VOAMASS and community-based settings.
Salary: $60,000- $67,000/year
Hours: Mon-Fri 9-5pm

The Integrated Community Support Services Expansion program is designed to improve access to mental health and substance use disorder care, strengthen coordination across essential services, reduce barriers to care, and expand comprehensive, person-centered supports for veterans and other individuals in Plymouth and surrounding communities.
As Program Manager, you'll be responsible for the implementation, coordination, oversight, and continued development of VOAMASS’s Integrated Community Support Services expansion program by ensuring the effective integration of behavioral health treatment, recovery services, community support services, and veteran services through VOAMASS’s “No Wrong Door” whole-person care model. This position develops and implements effective workflows across service lines, coordinates internal and external referrals, builds and maintains strong community partnerships, and ensures program goals, performance measures, grant requirements, documentation, and reporting expectations are achieved. In addition to program management responsibilities, the Program Manager carries a caseload and provides direct support to clients, helping individuals navigate services and connect to the appropriate behavioral health, recovery, community-based, and veteran resources.
Some of your critical responsibilities:
- Provide day-to-day leadership and operational oversight of the Integrated Community Support Services Expansion program in Plymouth.
- Lead implementation of the ICSS expansion, including establishing and maintaining workflows for intake, screening, referrals, care coordination, documentation, outreach, and follow-up.
- Promote integration between Behavioral Health Services, Veteran Services, Workforce Development, and other VOAMASS programs to improve coordination and reduce service fragmentation.
- Coordinate rapid access to behavioral health and community support services, with particular attention to individuals experiencing homelessness, veterans, justice-involved individuals, and people with mental health, substance use, and/or co-occurring disorders.
- Identify and address social determinants of health, including housing, food access, transportation, health insurance, identification documents, public benefits, employment, and other essential needs. The grant specifically calls for community support staff to help clients navigate these barriers while clinical staff provide integrated behavioral health treatment.
- Monitor program capacity, referrals, caseloads, service utilization, and participant engagement to identify barriers and ensure timely access to services.
- Collaborate with Data Analytics, Quality, Finance, Development, and program leadership to collect, analyze, and report required program and grant data and review regularly to identify service gaps and improve workflows.
- Support sustainability planning by identifying opportunities for third-party reimbursement, community partnerships, future grant funding, and integration of services into existing VOAMASS operations.
- Obtain and maintain Certified Application Counselor (CAC) certification to assist clients with applying for Medicare and Medicaid/MassHealth coverage, determining eligibility, navigating enrollment requirements, and tracking applications through completion.
The Program Manager role is about making a difference in the lives of the people we serve. You’ll bring the following qualifications:
- Bachelor’s degree in psychology, social work, health care administration, or related human service fields is preferred.
- Minimum of two years’ experience with social service organizations, understanding of and experience with the delivery of care at multiple levels; and experience with coordination and referrals for services.
- Excellent communication, organization, and computer skills
- Valid driver’s license; and reliable transportation are required.
- Desirable candidates will have prior knowledge of system navigation and resources available to individuals within the southeastern Massachusetts area.
