Full-Time | Flexible Hours | US-Based Only | Fully remote/work from homeLara Health is seeking a compassionate and experienced Medical Assistant (CMA/RMA) or Licensed Practical Nurse (LPN/LVN) to join our virtual care team. This is a fully remote role with flexible scheduling, ideal for someone who enjoys building strong patient relationships, providing ongoing support, and helping patients stay engaged in their care between office visits.This role is centered around monthly patient engagement, virtual check-ins, and supporting patients with chronic conditions through consistent, meaningful communication.Position HighlightsSchedule: Minimum 35 hours/weekLocation: Fully remote (U.S.-based only)Experience Required: Minimum 5 years caring for patients with chronic conditions such as diabetes, hypertension, cardiology, or primary careRole ResponsibilitiesConduct monthly virtual patient outreach and check-insBuild trusting, supportive relationships with patients over timeProvide patients with ongoing encouragement, education, and non-emergency supportHelp patients stay engaged with their care plans and health goalsDiscuss symptoms, medication adherence, home readings, and general health concerns as appropriateSupport patients in scheduling provider visits and following recommended care plansEscalate clinical concerns to the appropriate provider or supervisor when neededMaintain accurate and timely documentation in EMR and internal systemsCollaborate with providers and internal care teams to ensure continuity of carePerform other duties as assignedRequired Skills & QualificationsLicensure: Active Medical Assistant certification or LPN/LVN license requiredExperience: 5+ years in a clinical setting, preferably with chronic care or longitudinal patient supportLanguage: Bilingual English/Spanish strongly preferredExcellent communication skills with a warm, patient-centered approachStrong ability to engage patients consistently and build rapport over timeStrong organizational and time management skillsAbility to work independently and remotelyProficiency with computers, EMR systems, and virtual communication toolsMust have own device and reliable internet connectionFamiliarity with medical terminology and patient educationPassion for working with older adults and delivering preventive, relationship-based carePreferred ExperienceExperience with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or similar virtual care programsExperience supporting patients between office visitsComfort managing recurring monthly patient interactions at scaleFamiliarity with remote care coordination workflows and EMR-based task managementPlease NoteDo not apply if you do not meet the required qualificationsThis is a contract position and does not include benefitsNo agencies or recruitersOnline applications only - please do not call or emailOnly shortlisted candidates will be contacted
