HTS is seeking a full-time Physical Therapist to join our therapy team at a CMS 5-Star rated continuing care retirement community on northeast side of Fort Wayne, Indiana.

Details:

· Full-time, Monday–Friday, 30–40 hours/week.

· Respected facility offering diverse patient populations from short-term rehab, long term care, outpatient, memory care, assisted living and independent living.

· Well-equipped therapy gym with functional training spaces.

· Strong interdisciplinary support and welcoming therapy team focused on person-centered care.

Requirements

Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license. These qualifications include earning an advanced degree from a Commission on Accreditation in Physical Therapy Education-accredited physical therapist education program, passing the state licensure exam, and fulfilling the state’s requirements for licensure.
