You'll provide primary and chronic care to patients in Skilled Nursing, Assisted Living, and Independent Living communities with dedicated clinical and administrative support.

- Approximately 20 patient encounters per day
- No routine weekends
- Minimal call – approximately 1–2 overnight shifts per month
- In-house nocturnist team handles the majority of overnight needs
