Job Description:

Duties/Responsibilities

• Manage a caseload comprised of medically and culturally diverse inpatient and outpatient families  

• Assist families and patients by completing biopsychosocial assessments, identifying appropriate resources and referrals, assisting with discharge and transition planning, and serving as a liaison and an advocate between the patient and various systems 

• Demonstrate knowledge of child development related to diagnosis, assessment, intervention, and referral of infants, toddlers, pre-school and school age children, and adolescents to appropriate community agencies 

• Meet the needs of the families as identified, presented, or requested by the interdisciplinary team or physician 

• Document information in the medical record consistent with departmental standards 

• Maintain a working knowledge of a variety of referral sources and community resources appropriate for the patient population within the state of Texas 

Required Skills/Abilities

• Bachelor's degree or Master’s degree in social work from an accredited (CSWE) Social Work Program required 

• Texas State Board of Social Work Examiners (TSBSWE) licensure LBSW /LBSW-IPR  or LMSW required 

• Minimum 1 year of experience in health care social work preferred
