Treatment Nurse (LVN) – Full Time
Diablo Valley Post Acute is seeking a dedicated and compassionate Full-Time Treatment Nurse (LVN) to join our nursing team.
Starting Wage: $38–$39 per hour

General Purpose
The primary function of the Treatment Nurse is to ensure effective and efficient care is provided as prescribed by the physician and in accordance with facility policies and procedures.
The Treatment Nurse reports directly to the Director of Nursing (DON) and is responsible for providing care in accordance with current federal, state, and local regulations, as well as corporate standards, policies, and guidelines. The goal is to ensure the highest degree of quality care and safety is provided to our residents at all times.
Essential Responsibilities
Communicate regularly with residents and their families regarding Plans of Care, PT, OT, and other treatment protocols.
Provide excellent communication and strong customer service to residents and their families.
Maintain an acceptable standard of nursing practice and professional conduct.
Accurately and thoroughly document nurses' notes, reflecting the care provided and the resident's response to treatment.
Provide resident care and carry out physician orders, including medications and treatments within the scope of practice.
Perform treatments in a manner that protects each resident's dignity, privacy, and self-respect.
Accurately observe and assess wounds and pressure sores, promptly notifying physicians of lack of healing progress, deterioration, or signs and symptoms of infection.
Ensure appropriate wound-care management services are included in the resident's plan of care to promote healing, including special mattresses, nutritional supplements, and other appropriate interventions.
Assist in preparing and updating care plans related to treatment and wound-care needs.
Assist physicians and other healthcare providers during visits and examinations.
Communicate with physicians and other healthcare professionals regarding resident care, treatment, and condition.
Notify family members and attending physicians of significant treatment-related issues, including:
Sudden or marked adverse changes in skin condition or wounds.
Untoward responses to, unavailability of, or refusal of medications or treatments.
Treatment errors that present a significant risk.
Inability to timely obtain or administer prescribed treatment-related medications, equipment, supplies, or services when this poses a significant risk.
Participate in the orientation and ongoing training of nursing staff.
Report significant findings, changes in condition, and potential concerns to the RN Supervisor and/or Director of Nursing.
Provide patience, reassurance, and compassionate care to residents who may be anxious, upset, or in need of additional support.
Participate in facility surveys and inspections conducted by authorized government agencies as requested by the Administrator, Director of Nursing, or Assistant Director of Nursing.
Follow all applicable facility policies, procedures, regulatory requirements, and standards of nursing practice.
Supervisory Requirements
Assist with the overall supervision and coordination of nursing staff as directed by the Director of Nursing.
Support a positive, professional, and team-oriented nursing environment.
Qualifications
Education & Experience
Possession of a nursing degree from an accredited college or university.
Current and valid LVN/LPN license in the applicable state.
Two (2) years or more of nursing experience REQUIRED, preferably in a long-term care or skilled nursing facility.
Strong wound-care and treatment skills
