Purpose
To provide professional direct resident nursing care and, in conjunction with the PC Administrator and Clinical Director to supervise daily nursing activities performed by MED TECHs, PC Aides and CNA's. To promote and maintain a positive physical, social, and psychological environment for residents in the Personal Care Facility. To perform these duties in accordance with applicable current federal, state, and local regulations and standards, and as may be directed by PC Admin; and to perform these duties in accordance with the mission of Elm Terrace Gardens to promote the health and well-being of individual residents based on their functional abilities and potential.
Shift is 7p-7a schedule is Sunday, Thursday, Friday, Monday, Tuesday, Saturday full-time position with an excellent benefit package
Nursing Care Functions - Duties
- 1 Render and administer professional nursing services, including the administration of medications, in accordance with physician's orders and the physician-approved individual resident care plans; and provide direct nursing care as necessary to assigned residents.
- 2 Admit, transfer, and discharge residents as required; as directed, ensure that rooms are properly prepared to receive new residents.
- 3 As directed, notify families or responsible parties about resident expirations or transfers to acute care facilities.
- 4 Make rounds with residents' physicians as necessary and ensure their completion of record keeping.
- 5 Consult with physicians in providing care, treatment, diagnostic, and therapeutic services.
- 6 Notify physicians of incidents and changes in conditions.
- 7 Accurately complete, maintain, and verify drug inventories, medical forms, physician and consultant orders, reports, evaluations, and resident chart entries according to established policies, procedures, and guidelines.
- 8 Knows and complies with the facility infection control policies and procedures, including the blood- borne pathogen standard, infectious waste disposal, and such other standards as may be promulgated from time to time.
- 9 Ensure cleanliness and safety of all nursing service and treatment areas as well as cart audits
- 10 Follow established safety rules and regulations at all times, including fire safety procedures and disaster control.
- 11 Promptly report accidents, incidents, and unsafe and hazardous equipment or conditions.
Resident Assessment & Review Functions - Duties
1 Assist the PC Administrator and Clinical Lead Director of Personal Care as requested in conducting assessments of new residents and reassessments of current residents in order to identify nursing and medical problems and/or needs and to establish goals to address problems or needs.
2 Conduct such assessments according to documentation and time-frame requirements, including the Minimum Data Set and related documents, and according to individual resident care plan development.
3 Observe, document, and promptly report changes in residents' conditions to the PC Admin/PC Clinical Director , and/or resident physician, including changes in medical condition and levels of function, appetite, alertness, and behavior.
4 Participate as requested in the facility's interdisciplinary care program and quality assurance and related programs.
5 Attend scheduled care conferences, committee meetings, and such other sessions as directed by the PC Admin/PC Clinical Director
6 Develop and maintain a rapport with all facility services, residents, physicians, consultants, and
families to ensure an effective program of care and quality of life for residents.
Requirements
Qualifications
1 Must be a Licensed Practical Nurse in good standing and be currently licensed by the
Commonwealth of Pennsylvania Bureau of Professional and Occupational Affairs, Department of State; or
2 Must be a Licensed Practical Nurse in good standing with 2 years of supervisory experience in preferably a health care setting
3 Must be knowledgeable of nursing and medical practices and procedures and laws, regulations, and guidelines governing in Personal Care and or Assisted Living.
4 Must possess the ability to speak to and be understood by residents and staff, and demonstrate the ability to read, write, and follow oral and written directions
5 Must possess an agreeable and helpful attitude toward senior adults.
