Overview
As a Cardiology Nurse Navigator, you'll coordinate care for patients diagnosed with acute myocardial infarction (AMI), primarily focusing on NSTEMI cases. Your responsibilities include tracking patients from admission through discharge, coordinating diagnostic testing and consultations, facilitating transitions between care settings, and providing patient education. You'll monitor adherence to guideline-directed medical therapy, implement readmission prevention strategies, and participate in quality improvement initiatives. This role requires strong clinical knowledge in cardiology and excellent communication skills to work with multidisciplinary teams.
