Position Summary:

The Weekend Hospice RN is responsible for providing clinical coverage and coordinating referrals, evaluations, admissions, and patient care from Friday at 5:00 p.m. through Monday at 8:00 a.m. The position identifies patient and family needs, provides skilled nursing assessment and care, facilitates timely access to hospice services, and coordinates services in accordance with physicians' orders and the interdisciplinary team (IDT) plan of care to maximize the comfort and health of patients and families consistent with Peoples Hospice policies and procedures.

Requirements

Essential Duties & Responsibilities

1. Receive and respond promptly to referrals from hospitals, home health agencies, physicians, case managers, skilled nursing facilities, and other healthcare professionals during the assigned weekend coverage period.
2. Review medical records, conduct clinical evaluations, and communicate with physicians, patients, families, and referral sources to assess hospice eligibility and determine the appropriate next steps.
3. Assume responsibility for evening, weekend, and holiday on-call duty as scheduled and consistent with agency policies and procedures.
4. Initiate communication with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family.
5. Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care as indicated.
6. Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur.
7. Monitor physical care of the patient by instructing primary caregivers, volunteers, and employed caregivers.
8. Respond to medical or emotional crisis and/or death of patient and make home visit to provide direct care as appropriate.
9. Provide appropriate counsel at time of death.
10. Demonstrate familiarity with policies of the agency and rules and regulations of community, State and Federal bodies which relate to the delivery of home health/hospice services.
11. Accept other assignments as appropriate.

Required Qualifications:

1. Current Florida or multi-state license required.
2. Two years of RN experience is preferred.
3. Experience in acute, oncology, home health, and/or hospice setting preferred
4. Availability to work evenings, nights, weekends, and holidays.
5. Current CPR certification is required.
6. Valid driver’s license and current auto insurance is required.

Work Environment

While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. The employee works in varied residential environments based on assigned caseload.

Physical Demands

While performing the duties of this job, the employee is required to see, talk, or hear. The employee is regularly required to stand, walk, sit, reposition patients; use hands and fingers to feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 50 lbs.
