Position Overview

Reporting to the Home Care Director/Manager, the Registered Nurse provides age-appropriate nursing care to patients in their homes using standard nursing best practices, including:

• Assessment
• Planning
• Intervention
• Implementation
• Evaluation

The RN practices within the scope of practice defined by the New Hampshire Board of Nursing and works collaboratively with patients, caregivers, healthcare providers, and other members of the healthcare team.

The RN is also responsible for supervising Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) assigned to patients, in accordance with applicable federal and state regulations.

Essential Functions

 Develop and implement an appropriate Plan of Care (POC) based on comprehensive patient assessments, referral orders, treatment requirements, expected frequency and duration of services, and discharge goals.

• 1. Collaborate with the patient, healthcare provider, patient representative (when applicable), and caregiver(s) to establish individualized care plans that address patient needs, limitations, goals, and anticipated discharge environment.
• 2. Complete accurate and ongoing assessments and reassessments of patients.
• 3. Document nursing interventions, patient responses, education provided, patient/caregiver needs, capabilities, limitations, and progress toward established goals.
• 4. Provide and document appropriate patient and caregiver education in accordance with the plan of care and agency guidelines.
• 5. Engage patients and caregivers in meaningful education and self-management strategies as indicated by the care plan.
• 6. Communicate effectively with ordering providers and other healthcare professionals regarding the patient's current plan of care.
• 7. Coordinate the patient's plan of care with members of the Interdisciplinary Team (IDT) and other healthcare professionals.
• 8. Participate in team and patient care conferences and update short- and long-term goals based on interdisciplinary assessments and reassessments.
• 9. Participate in agency projects, committees, Quality Assurance/Performance Improvement (QAPI) activities, and other initiatives as requested and approved by the direct supervisor.
• 10. Attend agency in-service and continuing education programs.
• 11. Safely operate a personal vehicle while traveling throughout the Granite VNA service area for patient visits and/or meetings.

Additional Responsibilities

• 1. Supervise Licensed Practical Nurses and Licensed Nursing Assistants assigned to patients in accordance with federal and state regulations.
• 2. Coach, mentor, and precept new nursing team members and students.
• 3. Perform all job duties safely and responsibly to help prevent injury to yourself, patients, coworkers, and others.
• 4. Maintain professional nursing standards in all interactions with patients, caregivers, healthcare providers, and members of the healthcare team.

Requirements

1. Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)
2. One year of recent general medical/surgical or skilled/acute care nursing experience
3. Valid driver's license, reliable transportation and an acceptable driving record.
