Position Overview:
Works independently and in consultation with physicians and other health care providers across the health care continuum to ensure appropriate medication therapy for individual patients at home. Uses patient interviews and counseling, reviews the medical record and medication profile, consults with physicians, teaching, staff education, medication use evaluation, adverse reaction monitoring and resolves medication-related problems. Works closely with health care professionals and supportive staff of referral sources, home health nursing agencies, other ancillary provides to provide, and patient caregivers to deliver home therapies in a safe and user friendly manner. Documents service coordination efforts, patient progress and assessment, lab results, physician interaction, therapy intervention, and all pertinent communication according to company policies. Monitors patient care, sterile production preparation, and patient home delivery efforts of the pharmacy according to operation standards and regulatory requirements.Additional Requirements:
EDUCATION
- PHARMD-Graduate of an accredited pharmacy school
CERTIFICATION & LICENSURE
- RPH-Registered Pharmacist within 120 days
SKILLS AND KNOWLEDGE:
- Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations and laws and regulations.
- Knowledge and understanding of different Pharmacy practice settings, including narcotic delivery and procedures.
- Knowledge and ability to identify and employ pharmaceutical and medical terms, abbreviations and symbols commonly used in prescribing, dispensing, and record keeping of medications.
- Requires a basic working knowledge of legal requirements and accreditation standards including National Association of Boards of Pharmacy (NABP), The Joint Commission (TJC), Title XXII, United States Department of Homeland Security (DHS), Drug Enforcement Administration (DEA), Food and Drug Administration (FDA) and United States Pharmacopeia (USP).
- Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.

Pay Range is $84.32 to $118.05 / hourThe compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
