RAI Coordinator - Summit Place - Full time

Job Description

As a part of the Management team the RAI-MDS Coordinator reports directly to the Executive Director and is responsible for the sustainability of the RAI-MDS process and integration into daily practice across the home and maintaining the RAI process based on Home and corporate requirements and in alignment with Ministry standards.

Key Responsibilities:

• To provide leadership support to the and answer questions regarding interpretation of items and ensure homes are following the Canadian Institute of Health Information (CIHI) RAI, MDS 2.0 and RAPs Canadian Version Users Manual;

• To assist in implementing a Data Quality Program, develop and implement processes to ensure the integrity of data inputted into RAI tools, including data audits and inter-rater reliability.

• To ensure integrity of data input through ongoing education, auditing and support of all staff involved with the RAI process

• To analyze data and identify trends / issues.

• As a part of the management team assist in generating reports to inform and assist the home in determining appropriate reports for home committees.

• To assist in identifying evidence based quality improvements, work in collaboration with Regional and Home Management teams.

• To assist in determining appropriate assessments and monitor records to ensure ongoing identification of resident needs and opportunities to streamline with regards to data collection and reducing assessment burden

• To build/ develop/ grow any business relationships vital to success of RAI integration.

• To uphold and promote the organization's values and philosophy relating particularly to ethics, morality, and integrity as set out in Extendicare's Code of Conduct.

• To ensure all aspects of the RAI process are understood and followed by staff within appropriate timelines.

• To guide, direct and participate. in employee performance reviews and disciple.

• To support the home in staff orientation, on-boarding & coordination of RAI; MDS education to meet the needs of new/ existing staff.

• To complete quarterly regional reports including status of home's compliance of RAI process, audits, Resident outcomes and resources and overall progress for sustainability.

• To support PCC implementation and effective utilization related to RAI integration.
