POSITION PURPOSE:    

Reaches out in the workplace and various community settings to provide health education for individuals and groups, including the CGH Health Transformation Program. Collaborates with companies in CGH’s service area to develop wellness programs which improve employee health.

ESSENTIAL FUNCTIONS:

• Provides support and works with other organizations to help maintain high awareness of current public health changes and trends, both in community and in workplaces.
• Contacts businesses to promote implementation of a company wellness program – conducts employee health reviews, blood pressure, glucose, cholesterol and other health screenings, provides education and referrals for necessary health services.
• Offers health coaching services for individuals through Health Transformation Program. Develops and conducts group classes to teach health improvement skills related to reducing the risk of metabolic syndrome. Customizes care plans for HTP participants and monitors progress, conducting quarterly assessments. Uses coaching skills to educate and encourage individuals to adopt and maintain healthy lifestyle habits.
• Demonstrates experience in health communication and health education. Able to provide educational resources, including program presentations, speaker referrals, online health risk assessments, and educational websites.
• Organizes and conducts community screenings for cholesterol, blood pressure and blood glucose; provides education and referrals for necessary health services.
• Oversees planning for Growing Healthier Program and various community events.
• Coordinates Foundation programs, including Women’s Health, Strong Women Strong Bones, Eyeglasses for Kids/Adults and Geri-Fit.
• Works effectively with diverse clients from a variety of backgrounds in person, by telephone or email.
• Utilizes Microsoft Office Suite, including Power Point, Excel, Word, and Access.
• Maintains and troubleshoots equipment as well as orders supplies for screenings, programs.
• Sustains and improves clerical skills relating to telephone skills, communications skills (including effective listening, assertiveness, negotiation and counseling), as well as problem solving and decision-making.
• Performs other related duties as requested by manager.

SPECIALIZED KNOW-HOW & REQUIREMENTS:

• Three to five years’ experience working with clients/patients on healthy lifestyle management strongly preferred.
• Current licensure in the State of Ill. as LPN preferred.  Certificate in health coaching required or obtain within two years of hire.
• Demonstrates the knowledge necessary to provide care appropriate to any age-related needs of individuals in communities and employees of companies within CGH’s services area. Demonstrates knowledge of the principles of growth and development appropriate to the patient population served.  Able to assess and interpret data about the patient’s status in order to identify each patient’s needs and provide the appropriate care, including wellness and prevention care needed by the employee group.
• Good communication skills strongly preferred.
• Strong computer skills preferred.

Compensation Range

$16.58 - $24.87
