Monday-Friday 8-4:30.

Pay Range:

$22.90 - $34.35

Major Responsibilities:
  • Assist in the care management process.
    •     1)To assist the case managers with care management activities, including information gathering and correspondence with members and physicians in order to facilitate quality and timely services.
    •     2)Under direction of Care Manager, investigates and documents follow-up of complaints
    •     3)Coordinates and issues necessary letters, notifications and documents related to Care Management
    •     4)Prepare for department meetings and maintains minutes
    •     5)Contacts members and physician offices to gather needed information with direction of Care Manager
    •     6)Generate necessary correspondence to members and providers
    •     7)Schedule and arrange meetings and/or appointments with multiple attendees. Make decisions regarding revision of supervisor’s/staff’s schedule to accommodate meetings and appointments.
    •     8)
  • Data collection, analysis,and report production
    •     1)Assist with data collection for projects related to Care Management
    •     2)Create specialized reports and perform analyses of data.
    •     3)Produce charts and other graphics (bar charts, pie charts, pivot tables, etc.)
    •     4)Create advanced data presentations
    •     5)Create advanced Power Point presentations
    •     6)Create and format correspondence and various other documents—reports, policies, etc.
    •     7)Gather and organize data from multiple sources in support of management decision-making
  • Participate in education and referral related activities.
    •     1)Works with PHO staff to file, prepare correspondence, data entry, system documentation and other medical management related activities.
    •     2)Works with care manager in the education of physician office staff on case management procedures, AdvocateCare policies and procedures and benefit plan limitations.
    •     3)Communication with case manager, supervisor, manager, director, other health care professionals, and various staff in a positive fashion in order to promote member satisfaction, quality services delivered and resolution of issues.
    •     4)Identifies and maintains appropriate communication with supervisor involving problems and observations in course of daily operations.
  • Maintenance of department files.
    •     1)Maintain records and files within the department/unit.
    •     2)Create and set up filing systems for department/unit.
    •     3)Retrieves files and faxes and distributes them as requested
    •     4)Photocopies materials, as requested.
  • Preparation and dissemination of correspondence.
    •     1)Compose correspondence from notes and/or discussions with supervisor and/or staff
    •     2)Assembles and prepares material for department mailings, as requested.
    •     3)Type letters, memoranda, reports, and other types of correspondence from rough drafts or provided copy
