Trinity Health IHA Medical Group is seeking a compassionate and proactive Geriatric Panel Management Coordinator to support our Comprehensive Geriatric Program through advanced panel management, patient outreach, care coordination, and quality improvement activities. In this role, you will help older adult patients stay connected to their care team, close gaps in preventive and chronic disease care, and navigate transitions between visits and care management programs.
What You'll Do:
- Support geriatric providers through proactive panel management and patient outreach activities.
- Monitor chronic disease registries, preventive care gaps, and payer reports to identify patients needing follow-up services.
- Conduct outreach through MyChart messages, phone calls, letters, and other communication methods to engage patients in their care.
- Partner with Nurse Navigators and interdisciplinary care team members to coordinate services and support continuity of care.
- Schedule appointments, tests, and follow-up services based on identified patient needs and provider recommendations.
- Assist with onboarding new patients, distributing welcome materials, and fostering long-term patient-provider relationships.
- Support population health initiatives, performance improvement programs, and quality incentive measures.
- Review EMR and payer data to validate care gaps, support accurate documentation, and improve patient outcomes.
- Educate patients and families on preventive care, chronic disease management, and care plan adherence.
General Requirements:
EDUCATION: High School Diploma or GED. Successful completion of accredited/approved health program such as:
- Successful completion of IHA’s Panel Coordinator Onboarding Program
- Medical Assistant program (externship not required)
- Clinical Support Assistant/Certified Nursing Assistant/Patient Care Technologist
- Other clinical health certificate program as approved by the Quality Department Leadership
Bachelor’s or associate degree in health-related field or relevant experience is strongly preferred; examples would be Public Health, Health & Wellness, Health Education, Health Science or Health Administration.
MINIMUM EXPERIENCE: 3 or more years’ previous experience in panel management, care coordination, or similar role with a strong preference for prior Panel Coordinator and/or Medical Assistant background. Experience working with geriatric population and/or chronic disease management preferred.
