The primary purpose of your job position is to provide clinical occupation therapy evaluation and treatment for the
PACE Organizations' participants, on acute, restorative, or maintenance levels needed by the Day Center, Nursing
Home, or in-home setting.

Occupational Therapist

St Mary PACE - Day Center and In Home Care

Hours - Per Diem

We are seeking a compassionate and skilled Occupational Therapist to join our PACE team. In this role, you will provide comprehensive occupational therapy evaluations, treatment, and care planning for older adults in the day center, nursing home, and home-based settings. You will work collaboratively with an interdisciplinary team to help participants maintain independence, safety, and quality of life.

Key Responsibilities

  • Conduct occupational therapy assessments and develop individualized care plans.
  • Provide restorative, maintenance, and preventative therapy services.
  • Evaluate home safety needs and coordinate home modifications and adaptive equipment.
  • Train participants, caregivers, families, and staff on safe performance of daily living activities.
  • Document assessments, treatment plans, progress notes, and outcomes in accordance with regulatory requirements.
  • Monitor and maintain durable medical equipment inventory and utilization.
  • Collaborate with the interdisciplinary team to achieve participant-centered goals.
  • Supervise and provide guidance, competency assessments, and performance input for Certified Occupational Therapy Assistants (COTAs).
  • Support quality improvement initiatives and uphold organizational values, compliance standards, and participant-centered care.

Qualifications

  • Bachelor's degree in Occupational Therapy from an accredited program.
  • Current, active, and unrestricted Occupational Therapist license in the applicable state.
  • Minimum one (1) year of experience working with frail or elderly populations preferred; relevant clinical placement experience may be considered.
  • Two (2) years of occupational therapy experience preferred.
  • Strong communication, collaboration, and clinical decision-making skills.
  • Ability to work effectively in home, community, and clinical settings.
