As an RN, you will provide professional, patient-centered nursing care to a designated group of patients in the Family Medicine Clinic in Palm Beach Gardens, supporting patients throughout their care journey from initial contact through follow-up. This brand-new RN Triage role will support our busy primary care departments within Family Practice and provide an opportunity to make a meaningful impact on the patient experience. In this role, you will interact directly with patients, families, and visitors, providing education, answering questions, and helping connect patients with the appropriate care. You will assess symptomatic patients and determine the appropriate level of care based on their symptoms and underlying disease states, while promptly addressing critical concerns and abnormal laboratory results with providers to support timely evaluation and treatment.

A caregiver in this role works days from 7:30 a.m. – 4:30 p.m. or 8:00 a.m. – 5:00 p.m.

A caregiver who excels in this role will:

  • Assess health status by completing a nursing assessment. 

  • Perform telephone triage.  

  • Notify physicians/LIPs of abnormal findings.  

  • Establish a care plan in collaboration with the patient, family, and health care team. 

  • Collaborate with care coordinators to ensure smooth and effective transitions of care. 

  • Assist with specialized diagnostic and therapeutic procedures.  

  • Administer medications and treatments as ordered by physician/LIP and monitor patients for response.  

  • Initiate CPR and other emergency measures. 

  • Provide health education to patients and families. 

  • Develop, evaluate, adapt and document education to learning needs. 

  • Participate in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience and patient safety.  

  • Maintain professional growth and development in the nursing field and identify annual core competencies. 

  • Assess symptomatic patients and appropriately triage based on underlying disease state.

  • Address critical patient concerns and lab results promptly with the Provider.

  • Assist with peer reviews by providing pertinent information needed by the patient's insurance company.

  • Assist with patient teaching and education.

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited School of Nursing (ADN or BSN program) 

  • Current state licensure as a Registered Nurse (RN)  

  • Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross 

  • *In areas performing moderate sedation procedures and/or cardiac stress testing, must have current Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) or ALS through American Red Cross Certification within six months of hire

  • One year of recent experience as an RN

  • OR LPN, Medical Assistant and/or other unlicensed clinical healthcare experience if applicant is a current Cleveland Clinic caregiver in an ambulatory setting transferring into an RN role but remaining in the same current ambulatory Cleveland Clinic setting

  • OR a current Cleveland Clinic caregiver in a clinical role within the same specialty area transferring into an ambulatory area  

Preferred qualifications for the ideal future caregiver include: 

  • Bachelor’s of Science in Nursing (BSN)  

  • Internal Medicine background

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.

  • Requires corrected hearing and vision to normal range.

  • May requires some exposure to communicable diseases or bodily fluids.

  • Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects.

  • Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required for procedures.
