About the Role
As an RN Care Coordinator, you will assess transitional care needs, coordinate services across the care continuum, support care progression, and help patients transition safely from the hospital. You will partner with physicians, utilization review nurses, interdisciplinary care teams, patients, and families to address care needs and remove barriers to discharge.
Minimum Qualifications
- Active Registered Nurse (RN) license
- At least one year of nursing experience in an acute care setting
- Current Basic Life Support (BLS) certification
