Registered Nurse (RN) – Home Health
Full Time (Mon-Fri)/Chicopee Territory

Helping a patient recover at home takes clinical judgment, compassion, and a nurse who sees the whole person. At Elara Caring, you’ll build meaningful relationships with patients and families while delivering skilled care in the place they feel most comfortable.
As a Home Health RN, you’ll coordinate your patient caseload, assess changing needs, and work closely with an interdisciplinary team to help patients remain safe and supported at home.

How You’ll Make a Difference
- Complete admission and ongoing assessments to identify patient needs.
- Provide skilled nursing care according to physician orders and the individualized plan of care.
- Manage and coordinate care for your assigned patient caseload.
- Monitor patient progress, recognize changes in condition, and take appropriate action.
- Teach patients and caregivers how to manage care, support recovery, and maintain safety.
- Communicate with physicians and the interdisciplinary team to support continuity of care.
- Maintain accurate clinical documentation within regulatory requirements.
- Protect patient and staff privacy in accordance with HIPAA.
What You’ll Bring
- A current Registered Nurse license in the state of practice
- Graduation from an accredited nursing certificate, diploma, associate, or bachelor’s degree program
- At least one year of nursing experience in a clinical care setting
- Home health experience preferred, but not required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning, transfers, and equipment
- A valid driver’s license and acceptable driving record, or reliable transportation between patient homes and assigned work locations
