About LEAP
LEAP is a nonprofit organization dedicated to providing exceptional care and support to individuals with Intellectual and Developmental Disabilities (IDD) and Autism. Our mission is to empower individuals to live fulfilling lives by promoting independence, community engagement, self-determination, and high-quality, person-centered services.
About the Position
LEAP is seeking an experienced and collaborative Case Management Director to provide leadership for our adult Case Management services in Farmington, Maine.
As a member of LEAP's Senior Leadership Team, the Case Management Director has overall strategic and operational responsibility for the Case Management department. This position supervises and develops our team of Case Managers, oversees program quality and compliance, and helps ensure that individuals receive responsive, person-centered services consistent with MaineCare and Targeted Case Management requirements.
The Director may also carry a small caseload based on departmental needs. This is an opportunity for an experienced human-services professional to lead an established program while also helping LEAP prepare for the continued evolution of Maine's developmental-services system.
Requirements
What You'll Do
Lead & Develop the Case Management Team
- Provide day-to-day leadership and oversight of LEAP's Case Management department.
- Supervise, mentor, coach, and support Case Managers.
- Provide training, performance management, and professional development.
- Serve as a resource for complex cases and challenging situations.
- Build a culture of accountability, collaboration, professional growth, and person-centered service.
- Participate as an active member of LEAP's Senior Leadership Team.
Oversee Program Quality & Compliance
- Ensure Targeted Case Management services meet MaineCare Section 13 requirements and applicable state and federal standards.
- Oversee Organizational Representative Payee services consistent with Social Security Administration expectations.
- Monitor caseloads, documentation, service delivery, and program performance.
- Conduct regular audits and lead quality-assurance and continuous-improvement activities.
- Maintain effective processes and standards for high-quality Case Management services.
Support Individuals & Community Partnerships
- Promote individualized, person-centered services based on each person's needs, preferences, and goals.
- Support effective coordination of housing, employment, healthcare, transportation, social services, and other community resources.
- Build productive relationships with individuals served, families, state agencies, service providers, community organizations, and other partners.
- Promote independence, self-determination, meaningful community participation, and individual choice.
Help Shape What's Next
Maine's developmental-services system continues to evolve. The Case Management Director will contribute subject-matter expertise as LEAP evaluates and develops new or expanded Case Management, care-coordination, and community-resource services, including opportunities associated with Maine's Lifespan Waiver.
The Director may assist with program design, workflows, staffing considerations, compliance preparation, implementation planning, and the integration of newly developed services into the Case Management department when appropriate.
Minimum Qualifications
Candidates must have:
- A bachelor's degree from an accredited four-year institution with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing, or a closely related field.
- At least four years of experience working in the intellectual disabilities and/or autism field.
- Verifiable experience supervising employees who provide services to individuals with intellectual disabilities and/or autism.
- Strong leadership, communication, organizational, interpersonal, and problem-solving skills.
- The ability to work effectively with individuals served, families, employees, state agencies, providers, and community partners.
Preferred Qualifications
We are particularly interested in candidates who bring:
- A master's degree in a related field.
- Experience with MaineCare, Targeted Case Management, Maine's developmental-disability service system, and applicable regulatory requirements.
- Experience with program development, implementation, systems change, or organizational growth.
- Experience leading teams through changing regulatory or service-delivery environments.
- Knowledge of community resources and person-centered Case Management practices.
Clinical credentials are strongly valued. Preference will be given to candidates whose professional licensure or qualifications also allow them to serve as LEAP's Case Management Clinical Supervisor under applicable MaineCare Section 13 requirements.
What We're Looking For
The right person for this role is both a people leader and a program leader. You'll need to be comfortable supporting employees and individuals served while also managing compliance, monitoring program performance, solving operational problems, and participating in higher-level organizational planning.
We're looking for someone who can balance the details of today's Case Management operations with where Maine's developmental-services system is heading tomorrow.
Work Environment
The Case Management Director works across office, remote, residential, and community settings and travels periodically to meetings, homes, service locations, and other community settings. The position involves regular interaction with employees, individuals receiving services, families, service providers, state agencies, community partners, and LEAP leadership.
