Overview
The MDS Coordinator is responsible for maintaining the current MDS status of each resident in accordance with federal and state laws, scheduling residents for required assessments, and monitoring clinical systems for changes in resident conditions. Key duties include strategically setting assessment reference dates, completing MDS sections and CAAs accurately, coordinating interdisciplinary meetings, and electronically transmitting MDS data to the state. The position requires a current, unencumbered LVN or RN license in Texas, preferably with 2 years of long-term care experience and knowledge of Point Click Care system. Strong organizational skills and the ability to work under pressure while meeting deadlines are essential.
