Sign On and Relocation Support
This position is eligible for a $5,000 sign on bonus, paid in two installments:
- $2,500 after 90 days of employment
- $2,500 after 180 days of employment
Relocation assistance may also be available for qualified candidates.
What You’ll Do
As our Physical Therapist, you’ll play an important role in helping residents restore function, improve mobility, manage pain, and work toward greater independence. Your responsibilities will include:
- Evaluating residents and developing individualized treatment plans based on their needs, abilities, and goals
- Establishing realistic and achievable therapy goals based on each resident’s capacity and tolerance
- Providing physical therapy interventions designed to improve mobility, strength, function, and overall quality of life
- Evaluating resident progress and adjusting treatment plans as needed to help achieve therapy goals
- Collaborating with nursing, therapy, and other disciplines to develop and evaluate goals for each resident
- Educating residents and families on appropriate therapy methods and techniques
- Communicating resident progress, needs, and changes with residents, families, and other healthcare professionals
- Documenting resident condition, treatment, and progress in accordance with state and federal requirements
- Supporting MDS procedures and effective care planning
- Providing consultation and clinical supervision to Physical Therapist Assistants working within the facility
- Maintaining resident confidentiality and following HIPAA requirements
- Being dependable and maintaining regular attendance for scheduled shifts
- Supporting the Therapy team and helping wherever needed to provide excellent resident care

- A strong communicator who works well with residents, families, and other healthcare professionals
- Comfortable collaborating as part of an interdisciplinary care team
- Confident in assessing resident needs and developing realistic, individualized therapy goals
- Organized and able to maintain accurate and timely documentation
- Knowledgeable about HIPAA and resident confidentiality
- Familiar with state and federal requirements related to therapy delivery and documentation in a skilled nursing setting
- Dependable, positive, and committed to providing consistent, high quality care
- Someone who enjoys celebrating progress and helping residents reach their goals

Requirements
Education
- Bachelor’s, Master’s, or Doctorate Degree from a college or university with an accredited Physical Therapy program
Licensure
- Current Physical Therapist license in the state of employment
- Current CPR certification
Experience
- Six months of experience in a long term care environment preferred
Additional Requirements
- Knowledge of HIPAA and the ability to maintain resident confidentiality
- General knowledge and understanding of managed care
- Knowledge of state and federal requirements related to therapy delivery and documentation in skilled nursing facilities
- Regular and dependable attendance is required
- All Cascadia Healthcare employees must successfully complete and meet applicable state required background check requirements before working with residents
