Description
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients' homes.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
- Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and the interdisciplinary care team.
- Coordinate with our administrative team to develop a caseload within your preferred Buffalo-area territory.
- $82,000–$100,000 annually.
- Salary may increase based on patient volume.
- $45–$60 per follow-up visit.
- $65 per 4-unit evaluation.
- Your per-visit rate increases as your monthly patient volume grows.
- $60–$70 per follow-up visit.

- Ability to work independently in a home-based setting.
- Home health or geriatric experience is preferred but not required.
