Social Worker (Hospice)

Overview

Are you a dedicated Licensed Master Social Worker (LMSW) who believes compassionate care can change lives? At Sanctuary Hospice, we’re looking for a patient-centered professional to join our team.

 

As a Licensed Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times. 

 

Schedule: Part Time, Monday - Friday, 8:30 am - 2:30 pm

Coverage Area: Summit, Portage, Stark, Wayne, and Medina County

Responsibilities

• Be a Supportive Guide: Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Connect Hearts and Homes: Evaluate medical needs, home situations, financial resources, and community support.
• Provide Individualized Care Advocacy: Implement personalized social work plans and collaborate with your Bereavement Coordinator.
• Be part of a United Team Effort: Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide Counseling with Compassion: Provide empathetic counseling to patients and families.

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements:

AL, AR, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX: Current unencumbered license as a Social Worker from the State Board of Social Work

 

$34.50 - $35.50 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
