Iowa City Hospice is seeking a dedicated Part-Time Hospice Social Worker to join our interdisciplinary team and play a vital role in helping patients and families navigate the emotional, social, and practical challenges that accompany serious illness and end-of-life care. Enjoy a flexible part-time schedule of 20 hours per week during regular business hours, Monday through Friday between 8:00 a.m. and 5:00 p.m.

In this meaningful role, you will use your clinical expertise, advocacy skills, and compassion to provide counseling, psychosocial support, resource coordination, and guidance to patients and their loved ones. As a Hospice Social Worker, you will:

• Conduct comprehensive psychosocial assessments of patients and families.   
• Develop individualized care plans that address identified psychosocial needs and goals.  
• Provide emotional support, counseling, and crisis intervention to patients, families, and caregivers.   
• Facilitate discussions related to coping, grief, loss, advance care planning, and end-of-life concerns.  
• Connect patients and families with community resources, financial assistance programs, support services, and other available resources.  
• Advocate for patients and families to ensure their needs, wishes, and goals are respected.   
• Collaborate closely with physicians, nurses, chaplains, hospice aides, volunteers, and other interdisciplinary team members to provide holistic, patient-centered care.   
• Participate in interdisciplinary team meetings, care conferences, and care planning discussions.   
• Assist with discharge planning, placement options, and care transitions when appropriate.  
• Maintain accurate, timely, and compliant documentation within the electronic medical record.  
• Provide bereavement support and follow-up services as assigned.  
• Ensure compliance with Medicare, state, federal, and hospice regulatory standards.

Requirements

• Bachelor's Degree in Social Work from a CSWE-accredited program required; Master's Degree preferred.
• Current Iowa LMSW, LISW, or LBSW licensure preferred.
• Minimum one year of medical social work, counseling, or related healthcare experience preferred.
• Ability to work a part-time schedule of 20 hours per week during Monday through Friday business hours (8:00 a.m. to 5:00 p.m.).
• Valid driver's license and current automobile insurance required.
• Knowledge of medical terminology, healthcare systems, community resources, and social service programs.
• Strong communication, interpersonal, organizational, and time-management skills.
• Ability to work independently while collaborating effectively as part of an interdisciplinary team.
• Experience conducting psychosocial assessments, care planning, counseling, and patient advocacy.
• Compassionate, empathetic, and patient-centered approach to care.
• Ability to connect patients and families with appropriate community resources and support services.
• Demonstrated professionalism, sound judgment, and commitment to maintaining accurate and confidential documentation.
