Job Description:
Physical TherapistPerforms physical therapy evaluations, establishes plan of care to meet identified goals, provides treatments, evaluates effectiveness of treatments, and modifies plan of care as needed to achieve desired outcomes.EXPECTATION FOR ALL EMPLOYEES

POSITION SUMMARY
Performs physical therapy evaluations, establishes plan of care to meet identified goals, provides treatments, evaluates effectiveness of treatments, and modifies plan of care as needed to achieve desired outcomes.
PRINCIPLE DUTIES AND RESPONSIBILITIES
- Evaluates patients in terms of goals for physical therapy treatments/modalities
- Establishes plan of care, in accordance with physician’s orders, to achieve goals; implements appropriate treatment modalities
- Evaluates effectiveness of treatments, modifies care plan as needed to reach identified goals
- Assesses educational needs of patient/family/caregiver and provides education to meet identified needs
- Collaborates with interdisciplinary team during course of treatment and discharge planning to ensure overall hospitalization goals are met and patient is prepared to progress on the wellness continuum following discharge
- Completes documentation that accurately reflects care provided and meets clinical, organizational, and reimbursement requirements
- Oversees care provided by physical therapy assistants
At the request of the manager
- Participates in performance improvement activities
- Participates in peer evaluations
- Orients new hires, students
- Performs other duties as assigned
SKILLS AND ABILITIES
- Maintains regular, consistent and punctual attendance at the assigned job location
- Effective leadership skills
- Proficient in organizing, delegating, providing education to patients, families, co-workers
EDUCATION/CERTIFICATIONS/LICENSES/DEGREES
- Licensed as a Physical Therapist in the State of Oregon
- American Heart Association Healthcare Provider Basic Life Support (BLS)
EXPERIENCE
- At least one year experience as a physical therapist in acute care setting preferred

- Requires fine motor skills, adequate eye-hand coordination, and ability to grasp and handle objects
- Able to use proper body mechanics to assist patients in ambulating, transferring in and out of bed, chair or wheelchair
- May be required to lift up to 75 pounds
- Must use standard precautions due to threat of exposure to blood and bodily fluids
- Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone
- May require periodic use of personal computer
