Clinic Supervisor – Registered Nurse

Reports to: Executive Director
Supervises: Clinic Nurses (RN, CNA) and Clinic Clerk


About the Role

Under the supervision of the Executive Director, the Clinic Supervisor manages clinic flow and leads all staff in providing medical and nursing care, while collaboratively developing workflows and policies that ensure high quality standards. This role coordinates services with the primary care team and acts as the primary liaison. The Clinic Supervisor triages participants and serves as gatekeeper for daily clinic scheduling.

The role also provides skilled nursing care to participants: conducting assessments, implementing provider orders, developing plans of care, educating participants, caregivers, family and staff, and communicating participant condition to caregivers, primary care and the interdisciplinary team. It carries direct oversight of paraprofessionals (CNAs, CMAs, PCAs, HHAs, homemakers), including clinical education, training and observation, reporting requirements, and competency evaluation.


Qualifications

Education and Licensure

  • Associate's degree in Nursing required; BSN preferred
  • Active Registered Nurse (RN) license
  • BCLS certification required

Experience

  • Minimum 1 year working with the frail or elderly population
  • Minimum 2 years of nurse management experience in a hospital, nursing home or community-based setting

  • Skilled in managing personnel and a department
  • Skilled in developing workflows and processes
  • Excellent organizational skills
  • Excellent written and verbal communication skills
  • Broad knowledge of health care problems of the frail elderly
  • Skilled in physical assessment and triage
  • Knowledge of common safety hazards and precautions to maintain a safe work environment
  • Skilled in preparing and maintaining records, writing reports and responding to correspondence
  • Skilled in developing and maintaining department quality assurance
  • Skilled in building effective working relationships with participants, medical staff, staff members and family caregivers
  • Ability to react calmly and effectively in emergency situations

Medical Clearance and Competency

  • Medical clearance for communicable diseases and up-to-date immunizations required before direct participant contact
  • Position-specific competencies must be met prior to assuming participant care

Key Responsibilities

Leadership and Staff Management

  • Engage in team building; coach, mentor, instruct and train clinic staff to function independently
  • Provide orientation, initial and annual competencies; supervise and evaluate performance of Clinic RNs, Clinic Assistants and Clinic Clerk
  • Provide direct oversight of paraprofessionals (CNAs, CMAs, Pharmacy/Med technicians), including clinical education, training and observation, reporting requirements, and competency evaluations
  • Educate staff in the care and treatment of the elderly as needed
  • Ensure direct reports understand their responsibilities, authority and accountability, and work within their scope of practice

Clinic Operations

  • Manage clinic flow, appointment bookings, triage and gatekeeping in conjunction with primary care and staff, ensuring timely and appropriate care
  • Evaluate overall clinic effectiveness and implement change and quality improvement in collaboration with the Medical Director
  • Monitor clinic departmental expenditures and meet budgetary expectations
  • Respond to and triage incoming telephone calls; oversee scheduling of outside appointments for participants when necessary
  • Oversee order sign-off, medication ordering from pharmacy, and dissemination of orders and instructions; ensure implementation of provider orders
  • Coordinate participant care with outside contracted providers (nursing homes, assisted living, lab, oxygen, etc.)
  • Communicate with weekend and after-hours staff and follow up appropriately
  • Assist with hospital, nursing home and other institutional admissions

Clinical Care

  • Conduct in-person comprehensive initial, scheduled and unscheduled reassessments of PACE enrollees according to program protocols
  • Perform general skilled nursing care; perform or supervise administration of prescribed medications and treatments in accordance with nursing standards
  • Assist in treatment, examination and testing of participants
  • Educate participants, family and caregivers on care, medications, treatments and other aspects of health education
  • Manage and review participant records, charts and staff documentation; document in medical records according to PACE standards

Interdisciplinary Team and Communication

  • Participate actively in all interdisciplinary team and care planning meetings, communicating participant changes, collaborating on care planning decisions and coordinating 24-hour care delivery
  • Take the lead in communicating with the interdisciplinary team and community agencies
  • Keep the Executive Director and Medical Director informed of any issues or concerns

Compliance and Professional Development

  • Follow all PACE policies and procedures and OSHA safety guidelines
  • Protect privacy and maintain confidentiality of company procedures and information about employees, participants and families
  • Participate in and support Quality Improvement initiatives
  • Participate in continuing education and required staff training meetings; maintain professional affiliations and required certifications
  • Share rotating on-call responsibilities with all staff nurses
  • Act only within scope of practice
  • Perform other duties as required or requested

Work Environment and Physical Requirements

  • Interdisciplinary team model within an Adult Day Health Center and primary care clinic, with moderate noise levels and controlled temperatures
  • Manual and finger dexterity, eye-hand coordination, and ability to use department equipment
  • Ability to lift/carry up to 30 pounds using appropriate body mechanics, with reasonable accommodation if needed
  • Corrected vision and hearing within normal range, with or without reasonable accommodation
  • Ability to communicate by telephone and email with participants, customers, vendors and staff
  • May work under stressful conditions, with moderate pressure to meet deadlines and potential exposure to verbal aggression
  • Possible exposure to bodily injury risks, toxic substances, medicinal preparations, bodily fluids, communicable diseases and other conditions common in a healthcare environment
