Description

At Tufts Medicine, you’ll make an impact and build your career, alongside a community of brilliant, compassionate colleagues. Every member of our team plays an essential role in delivering healthcare that is accessible, empowering and centered on what matters most: the patient

Tufts Medicine Lowell General Hospital is seeking a compassionate and motivated Nurse Practitioner (NP) to join our multidisciplinary Addiction Medicine team. This position will provide comprehensive, evidence-based care to patients with substance use disorders across both the inpatient and outpatient settings.

The NP will play an integral role in the development and delivery of Addiction Medicine services, including inpatient addiction medicine consultations, management of substance use disorders, initiation and continuation of medication for addiction treatment (MAT), and ongoing outpatient follow-up through the Addiction Medicine Clinic.

Working collaboratively with physicians, nursing, behavioral health, social work, case management, and community-based organizations, the NP will help improve access to treatment, support recovery, reduce barriers to care, and facilitate seamless transitions from hospitalization to ongoing outpatient treatment.

A Schedule Designed for Work-Life Balance:

Enjoy a rewarding clinical career with a schedule that supports your professional goals and personal well-being.

· Flexible weekday schedule: Option to work a four- or five-day workweek, Monday through Friday, with standard hours of 8:00 a.m. to 5:00 p.m.

· No weekends or night call: Enjoy a predictable schedule without weekend or overnight call responsibilities.

· No observed holiday responsibilities: Enjoy the observed holidays with your family and friends.

Responsibilities:

1. Inpatient Addiction Medicine Consult Services

· Provide comprehensive Addiction Medicine consultations for hospitalized patients with substance use disorders, including opioid, alcohol, stimulant, and other substance-related disorders.

· Perform substance use assessments, including screening, diagnostic evaluation, withdrawal risk assessments, and evaluation of co-occurring medical and behavioral health conditions.

· Develop and implement individualized treatment plans for patients experiencing substance use disorders and withdrawal syndromes.

· Initiate, manage, and adjust medications for addiction treatment, including methadone, buprenorphine, naltrexone, and other evidence-based pharmacologic therapies.

· Assess and manage alcohol and opioid withdrawal, including appropriate medication protocols and ongoing monitoring.

· Provide education and counseling to patients and families regarding substance use disorders, treatment options, harm reduction, and recovery resources.

· Identify patients at risk for overdose and provide overdose prevention education, including naloxone prescribing and distribution.

· Collaborate with hospitalists, emergency medicine physicians, specialists, nursing, social work, and case management to coordinate comprehensive patient care.

· Facilitate referrals and connections to inpatient rehabilitation, residential treatment, outpatient programs, recovery services, and community-based resources.

· Develop individualized discharge and transition-of-care plans to ensure continuity of Addiction Medicine treatment following hospitalization.

· Provide clinical guidance and recommendations to inpatient teams regarding the management of patients with complex substance use disorders.

2. Outpatient Addiction Medicine Clinic

· Provide ongoing outpatient evaluation and treatment for patients with substance use disorders following hospital discharge and through direct outpatient referrals.

· Conduct comprehensive initial evaluations and establish individualized treatment plans based on patient needs and recovery goals.

· Initiate and manage medication for addiction treatment, including buprenorphine, naltrexone, and other appropriate pharmacologic interventions.

· Provide longitudinal medication management, monitoring treatment response, assessing adherence, and adjusting treatment plans as clinically indicated.

· Provide follow-up care for patients transitioning from inpatient hospitalization, emergency department encounters, and other healthcare settings.

· Monitor patients for recurrence of substance use, withdrawal symptoms, medication-related concerns, and other complications.

· Provide counseling, motivational interviewing, patient education, and harm reduction interventions to support recovery.

· Coordinate referrals to behavioral health services, counseling, recovery support programs, and community-based treatment resources.

· Collaborate with primary care providers and other specialists to address the comprehensive medical needs of patients with substance use disorders.

· Support timely access to outpatient treatment to reduce gaps in care and prevent avoidable emergency department visits and hospital readmissions.

· Maintain appropriate follow-up schedules and continuity of care for patients receiving Addiction Medicine services.

Qualifications

Education:

· Master’s degree in nursing (MSN) from an accredited Nurse Practitioner program.

· Current Massachusetts Nurse Practitioner license and authorization to practice.

· National board certification as an Adult-Gerontology Primary Care NP, Family Nurse Practitioner, or other applicable NP certification.

Experience:

· Prior clinical experience in Addiction Medicine, substance use disorder treatment, behavioral health, emergency medicine, hospital medicine, or primary care preferred.

· Experience managing patients with opioid use disorder, alcohol use disorder, and other substance use disorders preferred.

· Experience prescribing and managing medications for addiction treatment, including buprenorphine, preferred.

· Experience working in both inpatient and outpatient clinical settings is desirable.

· Strong clinical assessment, diagnostic, and treatment planning skills.
