Overview
As a Case Manager II, you will serve as the main contact for members receiving Long Term Services and Supports through Medicaid or State Funded programs. You'll conduct eligibility assessments, develop service plans, and coordinate resources. The role includes in-person home visits and collaboration with internal and external partners. Specializations are available in Intake, Non-IDD, IDD, and Children's departments.
