Position Summary
We are seeking a Marketer to join our team and play a dual role in referral source management and patient intake. This position relies on relationship-building skills to ensure we maintain close relationships with referral sources and build strong relationships with new referral sources.

Key Responsibilities
- Manage incoming referrals from hospitals, skilled nursing facilities (SNFs), case managers, and other partners.
- Generate new referral sources.
- Perform patient intakes and execute service agreement contracts as needed.
- Coordinate care initiation promptly to reduce delays and improve patient experience.
- Maintain accurate referral and intake trackers.
- Build and maintain strong relationships with referral sources through regular communication and check-ins.
- Generate new referrals from established referral sources.
- Identify opportunities for growth and assist with marketing initiatives as needed.
- Serve as a liaison between referral sources, patients, and internal care teams.
Qualifications
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage multiple priorities.
- Previous experience in home health, personal care, or case management preferred.
- Marketing or referral development experience is a plus (but not required).

Compensation
