What You'll Do

• Complete OT evaluations in patients' homes

• Develop individualized plans of care

• Help patients improve independence with activities of daily living

• Address dressing, bathing, grooming, toileting, meal preparation, and functional mobility

• Provide therapeutic interventions based on patient goals

• Assess home safety and fall risks

• Recommend adaptive equipment and home modifications

• Educate patients, families, and caregivers

• Monitor progress and adjust treatment plans

• Complete reassessments and discharge planning

• Coordinate with the interdisciplinary team

• Complete timely documentation using WellSky

What We're Looking For

• Active Maryland Occupational Therapist License Required

• Graduate of an accredited Occupational Therapy program

• Reliable transportation

• Strong clinical and communication skills

• Ability to independently manage home health visits

• Home Health Experience Preferred, but Not Required

• WellSky Experience is a Plus
