Job Title: Resident Care Coordinator or Assistant Wellness Director
General Description: The Resident Care Coordinator is responsible for the direct
supervision of caregivers under the direction of the Wellness Director and administrator. The Resident Care
Coordinator oversees all residents’ chronic medical condition assessment and
management.
Days and Hours on Duty:
Tuesday-Saturday 8:30am-5:00pm
Duties and Responsibilities:
• As directed, assist in the coordination of resident admissions.
• As directed, will ensure each resident’s physician report is complete.
• As directed, will complete a resident pre-assessment to determine
appropriateness prior to admission. This may include going out of the facility to
area hospitals, homes, or other assisted living facilities.
• As directed will prepare and complete a resident needs and service plan within
14 days after admission.
• Reviews all resident needs and service plans on a quarterly basis or as needed
basis if to address any change of condition.
• Communicates with the resident’s physician as needed.
• As directed manages any plans of action to correct deficiencies, and complete
medical forms, reports, evaluations, studies, charts, etc.
• Oversees all caregivers.
• Ensures the facility medication room for all centrally stored resident medications
is inaccessible to unauthorized personnel or residents.
• Reviews resident medications to ensure there is a written physician order for
each.
• Maintains communication with the pharmacies to ensure all resident
medications are managed.
• Adheres to all Title 22 regulations regarding incidental medical and dental care.
• Notifies the resident’s family and or physician regarding any changes in
condition.
• Can administer first aid as needed.
• Acknowledges all residents’ rights and will ensure staff does not violate those
rights.
• Ensures that any suspected or alleged elder abuse such as neglect, or
mistreatment of residents is reported.
