About the role

The Homeless Outreach Team (HOT) is a community-based, multidisciplinary service dedicated to supporting people who are experiencing homelessness or are at risk of homelessness.

  • Lead one of four primary Homeless Health Teams within HOT, providing nursing leadership and clinical oversight, with overall responsibility for budget and human resource management, while maintaining a strong focus on patient flow, patient care and service delivery.

  • Lead a community-based, multidisciplinary service dedicated to improving access to healthcare for people experiencing homelessness or at risk of homelessness through a person-centred, trauma-informed and harm-minimisation approach.

  • Work collaboratively with local health providers, housing services, government agencies and non-government organisations to deliver assessment, treatment, education, support, advocacy, referral and care coordination, and assisting clients to obtain and maintain safe and stable housing.

  • Oversee intake and referrals to the Homeless Health Service (HHS) through a "no wrong door" approach, ensuring people are connected to the right services regardless of how they enter the system.

  • Oversee daily health outreach clinics across multiple locations throughout the city, as well as street outreach, in partnership with government and community homelessness services, bringing healthcare directly to those who may otherwise face barriers to access.

  • Deliver brief interventions, health assessments, treatment, referral pathways and care coordination to support individuals in accessing mainstream health services of their choice.

About you

  • Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency (AHPRA)

  • Postgraduate tertiary qualification in a clinical specialty or management or a commitment to undertake postgraduate studies within 12 months; equivalent experience may be considered.

  • Demonstrated operational, financial, human resource and workplace health and safety management experience at a departmental level, with highly developed communication and interpersonal skills.

  • Comprehensive understanding of clinical governance and current nursing issues and demonstrated experience leading quality improvement initiatives, including the development, implementation, evaluation, and review of clinical policies, procedures, and service improvement strategies.

  • Proven ability to lead and support the delivery of evidence-based, patient-centred care within a complex healthcare environment, integrating theoretical knowledge, clinical skills, critical thinking and analysis into daily practice.
