Are you passionate about empowering individuals and families to improve their health, housing stability, and overall well-being? Join the Vancouver Housing Authority (VHA) as a Pathways Specialist and play a vital role in connecting our community members with essential resources and services!


We work closely with local governments and other community partners throughout Clark County to address issues of affordable housing and homelessness and help families break the cycle of poverty. As a Pathways Specialist, you’ll work with individuals at risk of adverse health or housing outcomes, conducting risk assessments, developing personalized plans, and helping participants navigate community resources. Your role is hands-on and impactful as you provide education, remove barriers to care, and support people in achieving self-sufficiency.


Key Responsibilities

  • Conduct intake and risk assessments to identify participant needs.
  • Provide one-on-one coaching to help participants improve their health and housing stability.
  • Connect participants with vital services, including healthcare, financial assistance, and transportation.
  • Collaborate with coworkers and community resources to help participants reach their goals. 
  • Assist participants in overcoming challenges such as paperwork, appointment scheduling, and compliance with housing programs.
  • Follow up with participants through home visits, phone calls, and visits in other settings where the participant can be found.
  • Maintain detailed records and contribute to program evaluation efforts.

Pay and Benefits

  • In compliance with the Equal Pay & Opportunities Act, the full wage range for this position is $23.57 - $33.00 per hour. Starting salary is determined by the required qualifications for the role and the candidate's relevant experience. 
  • Full-time, non-exempt,  OPEIU Local #11-represented position working in-person/on-site 8 am -5 pm, Monday - Friday.
  • Eligibility to enroll in our medical, dental, vision, life insurance, and disability insurance.
  • The benefits package also includes 5.58% paid into WA PERS by the employer, 12 company-paid holidays, and generous paid vacation time, as well as separate sick time accrual.

Requirements

  

  • Excellent interpersonal skills required to communicate effectively with residents, community partners, staff, and public officials. 
  • Must work effectively in a collaborative, team-based work environment, including the ability to positively receive and give constructive feedback. 
  • Provide excellent customer service to clients. Cultural awareness, sensitivity, and competency, including the ability to successfully work with and relate to individuals from diverse cultures and backgrounds. 
  • High School diploma or equivalent.
  • Minimum of two years’ experience in health education or chronic disease management.
  • Experience working with low-income, multi-ethnic populations preferred.
  • Bilingual preferred.
  • Knowledge of community resources in Clark County a plus.
  • Must have a valid driver’s license, access to a reliable vehicle, and have fewer than 3 moving violations in the last 3 years.
