A Brief Overview
Responsible for managing and coordinating population health initiatives identified through contract and program priorities, financial analysis, and value-based patient care needs. This includes coordination and completion of assigned projects, including project definition, assignment of task responsibilities, setting deadlines, monitoring progress etc. The Coordinator is an integrated member of the Population Health and Payer Strategy teams, which exist to provide strategic and tactical decision support to stakeholders at all levels of the organization. The Coordinator is responsible for managing and coordinating program outreach to patients as well as data collection and analysis of our patient populations. They are responsible for the communication, scheduling, and review of targeted patients within a population to aid in program success.
What you will do
- Review medical history, compiles and substantiates activity through documentation.
- Review patient's medical chart, including care management services, care coordination services, medical history, examination results, diagnostic tests, and other services for a subset of patients.
- Demonstrates competence and is proficient in validation of clinical data derived from a variety of data sources.
- Promotes and displays a positive customer oriented demeanor to patients, staff, and physicians to provide exceptional, high-quality customer service.
- Prepare and analyze data that supports value-based arrangements using tools and other reporting software to assist in data analysis and understanding.
- Working with the Population Health and Payer Strategies teams, evaluates data results and provides input to various committees/leaders, stakeholders, sponsors and team members at all levels.
- Demonstrate strong negotiation and influencing skills.
- Plan, schedule and coordinate events such as meetings, patient outreach, etc.

Required Education, License/Certification, or Work Experience:
- Associate's
- 1-3 years Related experience and/or training in operations, administration or healthcare.
Preferred Education, License/Certification, or Work Experience:
- Bachelor's Business Administration or Health Administration
